The caption on the reverse (not shown) reads, "Winning form. Chicago: Los Angeles Dodger Sandy Koufax shows winning form as he throws during game with Chicago Cubs here April 24th. Koufax struck out 18 Chicago Cubs setting a National League daytime strikeout record and tying his own overall National league strikeout mark. Dodgers won, 10-2." According to a label and the caption on the reverse, this photograph was taken on April 24, 1962.
